Shop with a Cop Application
The program is intended to help as many families in the Sweet Home School District as possible. The deadline to submit an application is November 25, 2024 at 5:00 PM.
Shopping Day is December 7, 2024. SHPD will contact you with shopping times.
Missing or omitted information will cause your application not to be accepted. A working telephone number must accompany this form. Eligible children are those between the ages of 4 through 12. Participation is limited. Preference will be given to those who did not participate last year. On the shopping day, your child must be present to shop. The child must live within the Sweet Home School District boundary. You must provide a copy of a utility bill to show proof of residency in the Sweet Home School District.
- The child must be present and accompanied by a parent or legal guardian on the shopping date. Purchases cannot be made for children who are not present. NO EXCEPTIONS. We will not reschedule children who miss their scheduled times.
- Each child shall be a resident of the Sweet Home School District. Enclose a utility bill or property tax statement to show proof of residency. You must be the Parent or Legal Guardian of the child.
- Participation is limited. Preference is given to those who did not participate last year.
- Merchandise CAN NOT be returned or exchanged. A list will be provided to the service desk for all items purchased by SHPD on your shopping date. Anyone attempting to return merchandise will be reported to SHPD and ineligible to participate in future assistance programs.
- Each child will be allowed to spend a specific amount and will not be allowed to go over that amount.
- Due to the number of children we take shopping, we will limit your shopping time with an officer to 30 minutes. It’s a good idea to have a specific list of items your child needs to have with you.
- Children will shop with officers. Parents/guardians will remain at SHPD until shopping is over, or you return within 30 minutes to pick your child up.
- Should your child be selected for the program, we will call you with a shopping date and time.
- On the shopping date the parent or guardian will need a photo ID.
