Teen Sharing Tree Application
The program is intended to help as many families in the Sweet Home School District as possible. The deadline to submit an application is December 6, 2024 at 5:00 PM.
First tags available on November 15th at 8:00 AM
Deadline to submit applications is December 6th at 5:00 PM
Deadline to return gifts is December 9th at 5:00 PM
Distribution of gifts is December 17th from 4:00 PM to 6:00 PM
One Application per Teen
- Eligible teens are those enrolled in 7th through 12th grade.
- The teen must live within the Sweet Home School District boundary - be enrolled in and actively attending school in Sweet Home.
- A working telephone number must accompany this form.
- Participation is limited. Preference is given to those who did not participate last year.
- Submit a utility bill or property tax statement to show proof of residency. You must be the Parent or Legal Guardian of the child.
